The Application for MALAS membership may be completed online or by mail. Membership fees are as follows: 

• Annual membership = $50

• Lifetime membership = $300

After completing this application, you will be directed to Paypal to submit your payment. You can also submit the downloadable application along with payment if you prefer to pay by check or money order.
